To register for the conference, you must complete the online form 

• Online Conference Registration (for both Members and Non-Members) - DOES NOT INCLUDE HOTEL RESERVATION OR FEES
• Printable materials are available for those who need to submit additional paperwork for payment processing (you must also complete the online form for your registration to be complete)

Within two weeks of submitting your registration form you will receive a confirmation email from the registration chair. The deadline for conference registration is Monday, May 2, 2011. Early conference check-in will be available Tuesday, June 7 from 4:00 p.m. to 6:00 p.m. On June 8th, conference check-in will begin at 9:00 a.m. and lunch will be served at 12:00 Noon, followed by the keynote at 1:00 p.m.

2011 Hotel Rates:
June 8-9 - 2 nights

• Single Occupancy - Per Room, Per Stay $220.00*
• Double Occupancy  - Per Person, Per Stay $110.00*

Additional Night

• Single Occupancy - Per Room, Per Stay $110.00 *
• Double Occupancy - Per Person, Per Stay $55.00 *

* Plus 13.75% tax for non-SUNY participants

• NYS Tax Exemption Certificate (ST-129) (11 kb pdf)

Hotel Reservations:

You can make reservations in one of two ways:

1. Call 800-Embassy and mention SUNYCDO
   • If you are sharing a room, it is best to call 800-Embassy, so you can list both names when placing the reservation.
     
     
2. Go to www.buffalo.embassysuites.com
   • Enter the dates in top right of page under "Check Availability" and hit "Go"
   • Type in "CDO" under "Group/Convention Code" box and hit "Continue"
   • The rate will appear as the package rate As SUNYCDO
   • Guests can continue through the screens until their confirmation number appears

NOTES:

• Reservations must be held with credit card
• Payment cannot be made via purchase order
• Cancellations must be received at least 48 hours prior to arrival
• We suggest making specific requests for non-smoking rooms, as the hotel has a designated smoking floor
• Valet parking is complementary for all conference participants
• The hotel also offers a complimentary airport shuttle upon special request; please notify the Lisa Allen if you would like to take advantage of this service

Attire

Conference dress is casual.

NCC Credits Available

Attention Counselors holding the NCC credential. This conference meets the NBCC criteria for continuing education units in the content area of Lifestyle and Career Development. To receive contact hours, complete the Contact Hour Verification Form and have it signed by a member of the conference committee. Please note other required documentation as specified on the Contact Hour Verification Form (30 kb pdf).
