Position Summary

Under the direct supervision of the Prevention Supervisor, the Caseworker is primarily responsible for providing home- based case management and family preservation services to identified families.  Responsibilities include assessment, referral and linkage, advocacy, casework counseling, case monitoring, and transportation.  

Essential Job Functions

To provide home-based intensive case management and family preservation services to the identified families for the purpose of:

1.       Decreasing risk factors impacting children in the home

 2.       Prevent the disruption of families due to child welfare issues.

Rural Health Network of South Central New York (RHNSCNY)

 

Position Announcement: Director, Chronic Disease Case Management.

 

The Rural Health Network of South Central New York is recruiting for an LMSW to lead and provide case management services for residents of Northern Broome and Tioga Counties, who have a chronic health condition and lack health insurance.  The Director, CDCM will maximize community services in support of uninsured adults with chronic health conditions.

Specific Responsibilities:  The responsibilities of the Director, CDCM include, but are not limited to, the following:

 

  1. Serve as the primary case manager for the Chronic Disease Case Management Project.  Maintain an active case load of thirty individuals. 
  2. Supervision of staff, student interns, AmeriCorps members and volunteers assigned to the Chronic Disease Case Management Project. 
  3. Development and administration of the Chronic Disease Case Management Project to achieve the two primary goals of the project: Improved health outcomes for those served, and determination of return on investment. 
  4. Budgeting and financial management of the Chronic Disease Case Management Project. 
  5. Promotion and marketing of Chronic Disease Case Management Project services. 
  6. Case management record keeping, data collection/entry and the timely submission of all required reports and evaluations. 
  7. Serve as staff liaison with, the two primary project partners, Our Lady of Lourdes Hospital and United Health Services. Provide effective, collaborative, inter-organizational service and leadership.   
  8. Ensure compliance with all project partner contracts, including the Appalachian Regional Commission, Our Lady of Lourdes Hospital and United Health Services.   
  9. Serve as a member of the RHNSCNY staff team and seek ways to continuously improve and support the work the RHNSCNY.  Seek and initiate tangible, effective ways to integrate and apply all RHNSCNY services to provide the maximum benefit to those served. 

Minimum Qualifications: New York State Licensed Master Social Worker (LMSW), and an acceptable history of relevant work experience.   Demonstrated micro social work skills of counseling as well as macro social work skills of program development preferred.  Valid N.Y.S. Driver’s License and use of private vehicle for work related purposes.
