Checklist

__ 1. Proposal application form and narrative form.

__ 2. Six collated copies of entire proposal and attachments -- unless otherwise stated in the grant requirements.

__ 3. Project budget (one page)

__ 4. Attachments from Community Organizations:

__ 5. Grant application has the original signatures of all partners. It is the responsibility of the applicants to secure signatures for all partners. All signatures must be original - facsimiles, emails, voice-mails may not be substituted.

__ 6. Submit grant on the original application form and narrative forms. DO NOT SCAN forms to change margins or section spacing. Your application will not be reviewed if spacing is altered on either the application or narrative form.

__ 7. Include only items requested in the application. Other materials will not be reviewed by the Coordinating Committee.

__ 8. Proposal must be submitted by the deadline: Friday, March 30, 2012 by 5:00 pm.

NO EXTENSIONS will be granted.
