• List specific measurable outcomes based on goals

Strategies/Actions

• Selected strategies or action steps to achieve objectives 

Evaluation Process

• Report actions and outcomes - Quarterly reports, data collection
• Assess findings and goals
• Adjust strategies or objectives to achieve amended goals & objectives

Levels of Partnership/Collaboration

• Partnership: shared resources; consensus decision-making; equality and mutuality; same goals, formal arrangements
• Collaboration: pooled resources (but what each brings to the effort is identifiable); negotiated decision-making; some shared goals and some separate goals
• Cooperation: help each other attain their own goals (though there may be some commonality)
• Networking: Sharing information; few or no common resources; little or no shared decision-making; informal agreements
