What health forms do I need to fill out?

New York state laws require Binghamton University's Decker Health Service Center to obtain required health documentation from all new students. The majority of graduate students need to submit several forms prior to course registration time in August.

If you successfully completed your undergraduate degree at Binghamton University and are immediately continuing as a graduate student, there is no need to resubmit required health forms.
