Coverage is available to individuals in a state funded graduate or teaching assistantship position earning a minimum stipend per academic year. Visits are subject to a co-payment, up to two visits per 12 month period, when you visit a participating provider and cover: initial exam, periodic exam, cleaning, bitewing ex-rays (4 per year), and up to two fillings per 12 month period. Completion of a PS-404G NYS Health Insurance Transaction Form is required to be eligible and is available on the human resources web-site or from the Human Resources Office.
