EMAIL, FAX or MAIL - If the online form does not work, please use the PDF registration form and return by email, fax or mail. You cannot register by phone as we need a paper copy of the registration for audit purposes.

WALK-IN - There is NO ON-SITE registration. The symposium is FULL!

HOW TO MAKE A PAYMENT

Payment must be made before the start of the program. All payments including credit cards will be processed by Monday, October 3, 2011. Any cancellations or nonattendance after this date will not receive a refund.

 CHECK:  Please make checks payable to: Research Foundation Award # 54401

PURCHASE ORDER:  Purchase Orders must state course name and registrant's name and provide complete billing information including contact name and phone number.

CREDIT CARD:  The University can only accept VISA or MasterCard. We will call you by phone after the form is received to get your credit card information for a secure transaction. DO NOT SUBMIT credit card information by email or fax as this is not secure.   Your purchase will appear on your credit card statement with the following notation: RESEARCH FOUND OF SUNY BINGHAMTON.

CONFIRMATION OF YOUR REGISTRATION

We will send confirmation and course information to registrants by e-mail, so please include your e-mail address on your registration form.

If you have not received confirmation seven days before the start of the program, call the Office of Industrial Outreach at 607-777-2154.

 CANCELLATION AND REFUND POLICY

All cancellations must be received in writing (email, fax or letter only – no telephone calls) to the Office of Industrial Outreach by 5pm Monday, October 3, 2011 to receive a full refund. No refunds for cancellations or non-attendance will be given after this date.
