  • Application for Transfer Credit
    In order to receive SOM credit for courses taken at another institution, students must first obtain SOM approval. Remember to complete and submit this form before you enroll in any course at another institution - otherwise, you may not receive SOM credit.

  • Closed Course Petition (PDF)
    Students willing to enroll in a closed course must complete and submit this form to the SOM Graduate Advising Office.

  • Course Request Form for Non-SOM Students (PDF)
    Non-SOM students wishing to enroll in SOM course(s) must complete and submit this form to the Graduate Advising Office. Non-matriculated students must complete and submit a Graduate School Nondegree Application Form (PDF), in addition to the Course Request Form.

  • Declaration of Candidacy (online form)
    In order to graduate and receive a diploma, a student must complete and submit this online form by the Drop Deadline of the semester prior to the one in which the student plans to complete his/her requirements for graduation. Check the submission deadline.

  • General Academic Petition (Available in Advising Office)
    Used for requesting exceptions to academic regulations.

  • Graduate Overload Petition for non-funded graduate students (Available in Advising Office)
    Non-funded graduate student must complete and submit this petition to request approval if you would like to take more than 18 credits in a given semester.

  • Graduate Overload Petition for funded graduate students (Available in Advising Office)
    Funded graduate student must complete and submit this petition to request approval if you would like to take more than 18 credits in a given semester.

  • Internship Contract (PDF)
    To receive academic credit for internship work, you must complete and submit this form to Academic Advising. See internship guidelines (PDF) for policies and procedures regarding internships.

  • Late Add/Drop Petition (Available in Advising Office)
    Use this petition to request an add or drop after the published deadlines. Check the Academic Calendar for the Drop/Add deadline.

  • MS/Accounting Core Course Review Policy (PDF)

  • Request to Register for Independent Study (Available in Advising Office)
    Contract describing the purpose/learning objectives of the student's independent study, the intended methods to achieve such objectives and the means the faculty supervisor will use to evaluate the completion of such objectives. Must be submitted along with a completed Variable Coursework Registration Form.

  • Variable Credit Coursework Registration Form (PDF)
    Students wanting to register for an independent study, or other variable credit course must complete and submit this form along with any other necessary forms or contracts to the SOM Graduate Advising Office.
