Responsibilities of Students Authorized to Receive Academic Accommodation Letters

Submit a "Request for Academic Accommodation Letters" form to SSD each semester in which academic accommodations for equal access are needed.
Pick up your academic accommodation letters from SSD office.
Give accommodation letters to your professor(s) each semester.
Schedule an appointment to meet with your professor(s), review the academic accommodation letter(s), and discuss how the accommodation(s) will be implemented.
Contact SSD with any questions or concerns.

Responsibilities of Students Authorized to Receive Testing Accommodations

Schedule appointment to meet with your professor, present academic accommodation letter, and discuss testing accommodations. Discussion must take place at least one week prior to a scheduled exam to allow professor(s) sufficient time to arrange for testing accommodations.
Throughout the semester, you must inform the professor at least one week prior to each scheduled exam and quiz if you will be using the testing accommodation(s).
If you do not give the professor sufficient time to arrange for a testing accommodation (one week notice), the professor may not be able to accommodate your request.
You must report on time for a scheduled exam. If you arrive late to a scheduled exam, the predetermined ending time for the exam remains the same.
If you and the professor agree to an unproctored testing location, you understand unauthorized individuals are not to be present in the room during the exam.
If you have been authorized for a test reader or test scribe you must contact SSD Assistant Director Carol Hall to make the arrangements. You must provide SSD a week’s notice before a scheduled exam wherein reader/scribe services are needed or such accommodations may not be possible.
You agree to follow the rules listed in student handbook regarding Student Academic Honesty Code.

Responsibilities of Students Authorized to Receive Note Taker Services

You must contact SSD Assistant Director Carol Hall, work with her in following the established guidelines and instructions, and select your note taker.
You must attend every class for which you are receiving note taker services. You are not entitled to notes for classes or portions or classes that you do not attend.
The SSD office reserves the right to contact a student’s professors to clarify attendance.
Notes are for your own study purposes. They are not to be shared, copied, or given to others.
If you are experiencing difficulties with the note taker accommodation, you should promptly contact Carol Hall for help with resolving any difficulty.

Responsibilities of Students Authorized to Tape Record Classes

Students authorized to make an audio recording of class lectures, discussions, or tutoring sessions for later review understand this accommodation is for their own personal study and should not be shared or used for any other purposes.
Sharing, copying, or reproducing a digital recording without consent of the professor and other individuals on the recording is considered a violation of the student code of conduct and may result in severe penalties.

Responsibilities of Students Authorized to Receive Interpreter Services

You must submit your course schedule to SSD as soon as you have registered for classes.
You must provide SSD with dates, times, and locations of service needs for out-of-class activities as soon as the student becomes aware of them.
Advance notice of specific sign language interpreter needs is critical to SSD's ability to recruit qualified interpreters for the dates and times during which the service is required.
You must give SSD advance notice if you are unable to attend a class or activity for which an interpreter has been scheduled.
