2014 Decker Student Health Service Center (DSHSC) Survey: General Information

During the Spring 2014 semester we are gathering information from students who may have been ill, and did not use the DSHSC. We hope to gain information regarding barriers students may face when attempting to use our services. Aggregated responses provide information that will help focus our improvement efforts.

Which students will receive the survey?

A random sample of currently enrolled students who did not have an illness-related visit to the DSHSC since 2011 will receive the survey.

Why should I participate?

Participation is voluntary and the survey can be discontinued at any time. Responses help shape the focus of our improvement efforts and provide us with information that is not otherwise available to us. A high response rate is required so we will follow up with students who do not respond.

Will there be repercussions based upon how I respond?

Individual responses are confidential and used only to create aggregated responses. The confidentiality of responses is protected by federal laws that prohibit usage of the information beyond basic statistical analyses.

Will the survey take a long time to complete?

We have tested it and found it takes about 3 minutes to complete.
