Forms for Degree Completion

Graduate Application for Degree (Master's and PhD)

Used to declare that you plan to graduate in a given semester. Master's and Doctoral degree students must complete this online survey during the semester in which they plan to graduate. This serves as a declaration of intent to graduate and an application for degree completion. It can be accessed through the Graduate School's website.  http://www2.binghamton.edu/grad-school/new-and-current-students/academics/index.html

Proposed Course of Study/Graduate Check (Master's only)

Used to check progress toward degree and, at time of graduation, to show which courses are being counted toward your degree. Available at the Graduate Coordinator's office and on-line. Graduate Coordinator submits this form, along with your "Recommendation for Award of Master's Degree", to the Graduate School at the time of graduation. Each Master's program has its own form.

Recommendation for Award of Master's Degree

Used at time of graduation to submit proof of degree completion to the Graduate School. Available at Graduate Coordinator's office. Graduate Coordinator submits this form, along with your “Proposed Course of Study/Graduation Check”, to the Graduate School at the time of graduation. Each master's program has its own form.

Recommendation for Award of Doctoral (PhD) Degree

Used at time of graduation to submit proof of degree completion to the Graduate School. Paper only. Available at Graduate Coordinator's office.


Forms for PhD Students

Principal Adviser/Guidance Committee (.pdf, 829kb)

Used generally during the first year of PhD study to designate the student's principal doctoral adviser and other faculty who will serve on the student's dissertation committee.

Statement of PhD Research Interest (.pdf, 707kb)

Used, along with the Principal Adviser/Guidance Committee form, to state the research interests which the student expects to pursue, and proposed members of the PhD Guidance Committee.

Learning Contract (Recommended Format) (.pdf, 39.8kb)

Paper document used to show the recommended format for the Learning Contract, which outlines the requirements for passing the Comprehensive Exam.

Recommendation for Admission to Candidacy

Used generally after students have completed their Comprehensive Exams to declare to the Graduate School that a student is being admitted to candidacy for the doctoral degree.

Independent Study Registration (.pdf, 489kb)

Used to register for courses which are considered "independent study".

Late Add/Change/Withdraw (.pdf, 118.9kb)

Used to add, drop or change a course after the add/drop deadline, or to change the Grade Option.

Certification of Full-Time Status (for Students Not Receiving a Tuition Scholarship) (.pdf, 61.9kb)

Used in the fall and spring semesters by non-funded students who must maintain full-time status, who have completed all of their course work and are only registering for one credit of thesis, project or dissertation.

Application for Transfer Credit (.doc, 46kb)

Used to request the transfer of graduate credits from other institutions toward a degree at Binghamton. Download the form for completion and forward with any necessary information to your program's graduate director.
